Client Care Manager – Premier Home Care

We are currently seeking a passionate and dynamic Client Care Manager to join our team.
Position Overview:
The Client Care Manager will be responsible for managing and nurturing client relationships, conducting assessments, driving sales growth and expanding our customer base via relationship building and networking within the public and private health arena to increase the company’s presence in the community. This role requires a client-focused, organized and results-driven team leader with a strong understanding of the home care industry and a passion for helping others in an agile working environment.
Key Responsibilities:
- Client Assessments: Conduct initial assessments for new clients, determining their care needs and creating individualized care plans that align with their goals.
- Client Relationship Management: Work on all aspects of the client life cycle, from prospect to conversion. Build and maintain strong relationships with new and existing clients and their families to ensure high satisfaction and retention. Address client concerns promptly and professionally, ensuring their needs are met so to create a positive customer experience.
- Collaboration with Care Teams: Work closely with the Care Coordinator team, supervisors, and other caregiver team members to ensure seamless service delivery and client satisfaction.
- Reporting & Documentation: Maintain accurate records of client interactions, sales activities, and assessments. Provide regular updates to senior management on performance and business referral initiatives.
- Sales & Business Development: Develop strategies to grow the company’s client base and revenue by strengthening & nurturing referral channels in the hospital, rehab, public health and community arena. Collaborate with other colleagues to seek out new business opportunities through networking, promotion, and marketing initiatives.
- Marketing & Networking: Represent the company at local events, community functions, and professional networks to increase brand awareness. Participate in and implement marketing strategies to attract new clients and build the referral channel.
Qualifications:
- A passion for helping others and making a positive impact on the community.
- Bachelor’s degree in Health Services (LPN preferred), Social Work, Business Administration, or related field.
- Proven experience in client relationship management, sales, or business development, ideally in the healthcare or home care industry.
- Strong knowledge of home care services, industry regulations, and client care processes.
- Excellent communication, interpersonal, and organizational skills.
- Self-motivated, goal-oriented, and able to work independently as well as in a team environment.
- Accepts and thrives within a multi-faceted role within a SME environment
- Willing and available to work on the road and from home.
- Bilingual (verbal and written) is an asset.
- Has a driver’s license and a vehicle available at all times.
- Available to work flexible hours, some evenings or weekends occasionally.
